CRS 126 Medical Documentation for Coders

Trains students to read and interpret medical documentation in order to extrapolate data for the purposes of properly coding diagnoses and services rendered.

Outcomes

Upon successful completion of this course, students will be able to:
Demonstrate an understanding of the different elements of medical documentation. Explain the Current Procedural Terminology (CPT) Evaluation and Management (E/M) coding guidelines and how they affect code selection. Demonstrate competency in extrapolating and interpreting data from medical documentation using the CPT E/M guidelines in order to make correct code selections for both diagnosis (ICD-10) and procedures and services (CPT/HCPCS). Demonstrate competency in the use of Time-Based Coding using the CPT E/M Guidelines in order to make correct code selections for both diagnosis (ICD-10) and procedures and services (CPT/HCPCS). Demonstrate competency in extrapolating and interpreting data from medical operative notes in order to make correct code selections for diagnosis (ICD-10) and procedures and services (CPT/HCPCS).
